Fidelity's dental plan helps eligible employees pay for a variety of dental services (including preventive, basic, major restorative, and orthodontic treatment) performed by a licensed dentist. The dental plan offers an optional Preferred Dental Provider (PDP) feature, which enables employees to take advantage of negotiated rates with select dentists.
Fidelity's disability plans provide you with substantial protection for your earnings should you become disabled as defined by the Plan(s). If you are a regular employee who is regularly scheduled to work 30 or more hours per week, you are automatically covered under the Short-Term and Core Long-Term Disability Plans as of your first day of active employment. In addition, eligible employees may purchase Supplemental Long-Term Disability coverage.
Recognizing the importance of fitness as part of a healthy lifestyle, Fidelity offers eligible employees an annual reimbursement for a fitness center membership or a qualified fitness-related expense. Employees may participate in the Fitness Reimbursement Program in addition to receiving any discounts or reimbursements that may be offered by their medical plan.
Fidelity sponsors a voluntary Group Long-Term Care Insurance Plan for eligible employees and their eligible family members underwritten by John Hancock Life Insurance Company. The plan is intended to complement medical coverage by providing benefits for extended care resulting from long-term, chronic illnesses or injuries that Medicare or other medical coverage is not designed to cover. It covers skilled, intermediate, and custodial care in your home, an adult day care center, an alternative care facility, or a qualified nursing home.
Contributing to either the Health Care TaxSaver Account or the Health Care TaxSaver Account Limited (flexible spending accounts) allows you to set aside money on a pretax basis to reimburse yourself for certain eligible medical expenses, thereby reducing your taxable income.

The Health Management Center provided by WebMD -- Eligible employees (as well as spouses and adult children who are 18 and older and are enrolled in a Fidelity medical plan) can access the Health Management Center, an information-rich website that includes tools and resources designed to help you and your family live healthy lives and make more informed healthcare decisions.
Health Dialog -- Eligible employees and their eligible dependents have access to Health Dialog anytime, day or night. The Health Coaches at Health Dialog are licensed health care professionals who can answer general health questions, help you prepare for your next doctor's visit, discuss symptoms, a diagnosis or prognosis, or determine appropriate ways to get care for a particular problem.
Fidelity has life insurance coverage to help eligible employees plan for their survivors' financial security. Fidelity automatically provides eligible employees with Core Life Insurance Coverage which includes Employee Life Insurance and Dependent Life Insurance, all at no cost to the employee. Fidelity also provides all eligible employees (regardless of hours worked) with Business Travel Accident Insurance at no cost. Employees may also elect to participate in the Variable Life Insurance Plan.
Fidelity offers eligible employees a range of comprehensive coverage options. Eligible employees can enroll in the HealthFlex Plan, the HealthFlex High-Deductible Plan (Preferred Provider Organizations - PPOs), or, in some locations, a Health Maintenance Organization (HMO). All medical plans provide coverage for eligible expenses incurred for emergency care, medical services in the doctor's office or hospital, and prescription drugs. With the PPOs, you have the flexibility to seek care from an extensive national network of preferred medical providers or from any other provider of your choice. No referrals are necessary.
